Requesting Health Information

The Peterborough Clinic respects and maintains the confidentiality of every patient’s health information and its custody according to government legislation.

This is an uninsured service and is not covered by OHIP. Fees are determined based on the type/size of records requested. Payment and consent must be received prior to releasing any records.

If the physician you are requesting records for is still practicing at The Peterborough Clinic, please contact the physician’s office directly to obtain a copy of the medical records.

Physician’s phone and fax numbers are located on our website by clicking on ‘For Patients’ and then selecting ‘Physicians’.

Patient/Family Requests

If your physician has retired/closed their practice, you can request a copy of your medical records by clicking on the following link:

Patients 16 years of age and older must sign their own consent form, and payment must be received prior to releasing any records. Once this has been completed, a digital copy of the records will be uploaded to MedChart’s secure portal. You will receive an email notification stating that the records are available. Instructions on how to download a copy of your records will also be provided in this notice.

Third Party Requests

Physician’s offices, insurance companies, lawyers and other third parties can request a copy of a patient’s medical records for a physician who has retired/closed their practice by clicking on the following link:

A signed consent from the patient or the patient’s substitute decision maker is required.

Third parties will be required to pay a processing fee to MedChart for the request. If you would like to avoid the processing fee, please have the patient request a copy of their own records.
